CYO Youth Basketball League — Participant Waiver & Release of Liability

In consideration of my child's participation in the CYO Youth Basketball League ("the League"), I, the undersigned parent or legal guardian, hereby agree to the following:

1. Assumption of Risk. I acknowledge that participation in basketball and related activities involves inherent risks, including but not limited to physical injury. I voluntarily assume all such risks on behalf of my child.

2. Release of Liability. I hereby release, discharge, and hold harmless the CYO, its officers, directors, coaches, volunteers, and affiliates from any and all liability for injury, loss, damage, or expense that may arise from my child's participation in league activities.

3. Medical Authorization. In the event of an emergency and if I cannot be reached, I authorize league personnel to seek medical treatment for my child as they deem necessary.

4. Photo Release. I grant permission for photographs and videos taken of my child during league activities to be used by the CYO for promotional and educational purposes without compensation.

5. Code of Conduct. I agree that my child and I will adhere to the CYO Code of Conduct. Failure to do so may result in suspension or removal from the league.

6. Acknowledgment. I certify that my child is medically able to participate in physical activity and that the information provided in this registration is accurate to the best of my knowledge.
